Community-acquired bacterial meningitis (CABM) is a life-threatening condition requiring prompt antibiotic therapy to improve outcomes. A recent study conducted a systematic review to assess the impact of time to antibiotics (TTA) on death and neurological impairment in adults with CABM.

Data from various hospitals in Australia, Denmark, and the United Kingdom were analyzed to determine the association between treatment delay and patient outcomes. The study found that delayed initiation of antibiotics significantly increased the risk of death due to CABM, with each hour's delay associated with a 10% increase in mortality odds. Moreover, neurological impairment was also more likely with delayed treatment, emphasizing the importance of early intervention.

The analysis included individual patient data from different cohorts, showing that after a 2-hour delay in antibiotic therapy, the odds of death due to CABM more than doubled.

Notably, patients treated within the first hour had improved mortality rates compared to those treated after 5 hours, indicating the critical nature of prompt treatment. The study also highlighted the impact of microbial pathogens on patient outcomes, with certain bacteria like Streptococcus pneumoniae associated with higher mortality rates. Additionally, the presence of impaired consciousness and comorbidities were significant predictors of adverse outcomes.

The findings underscore the urgency of initiating appropriate antibiotic therapy in patients suspected of CABM. The study's results have implications for clinical practice, suggesting that treatment should ideally be administered within 2 hours of presentation to improve patient outcomes. Moreover, the data emphasize the need for early recognition and management of CABM to reduce mortality and neurological sequelae. By addressing the time to antibiotics, healthcare providers can potentially enhance the care and prognosis of patients with this life-threatening condition.
